Q: What types of insurance do you accept?
A: Most popular insurance plans are typically accepted, including Medicare, Anthem, Medical Mutual of Ohio, Aetna, Tri-Care/Champus, Caresource, Klais, Ohio Preferred Network, United Healthcare, Hometown, Cigna, Emerald, Primnet and Summa. However, given variations in most plans, we provide no-charge benefit verification prior to each patient's first visit.

Q: I think I may need one of your services. How do I proceed?
A: Start with seeing your Primary Care Physician. If he/she thinks you would benefit from any of our services, have them fax us a prescription or have them write you a prescription.  Then just call us so we can determine insurance coverage and schedule your first appointment.

Q: I'm not able to secure my own transportation to/from your clinic. Can you transport me?
A: Yes, we provide transportation from your residence, to the closest LifeLine clinic and back to your home for a nominal fee.

Q: How far can you drive to pick me up if I need transportation to one of your clinics?
A: Each LifeLine Partners clinic services it's own 25 mile radius. Pickup is computed between the patient's residence and closest LifeLine Partners clinic.

Q: After completing my Physical or Pulmonary Therapy sessions, do you offer a program that lets me continue my exercise program?
A: Yes, so that these patients don't regress to their pre-therapy level, LifeLine Partners offers a Maintenance Program. For $20 a month, patients can use any LifeLine clinic to perform their exercise routine, including use of all equipment, including treadmills, recumbent bikes and weight machines and supplemental oxygen. This a month-to-month membership with no long-term contract.
